Self-management support is the assistance caregivers give to patients with chronic disease in order to encourage daily decisions that improve health-related behaviours and clinical outcomes.

As part of our Co-creating Health work, two programmes were established, ADP and SMP, where patients tutors delivered training alongside clinicians. Here clinicians discuss how the programme has changed their practice and patients talk about their experiences of the programme and how it has made a difference to their quality of life.
